The Galveston County Health Access Program with its many member organizations and numerous individuals is involved in many projects around Galveston County. Below is a list of some of these projects. If you would like to learn more or get involved in one of these projects please contact us.

S.M.A.R.T. Family Literacy Project - The SMART Project in
Galveston recruits and trains volunteers who read with children and families in
schools, shelters and at community-wide literacy fairs.

St. Vincent's House WelCare Initiative - St. Vincent's House is expanding their free clinic into a community-wide
“WelCare Initiative.” The Initiative incorporates the WelCare philosophy of wellness and health into St. Vincent's
clinical activities; promotes learning about and adopting health promoting behaviors through programs designed for the
target population and environment; promotes disease risk reduction through innovative approaches that involve the
individual and the community; increases utilization of appropriate health care services among the target population;
and expands St. Vincent's role in an integrated community-wide continuum of care that services the uninsured, especially
those that live in low-income, predominantly minority neighborhoods.
Diabetes Educational Classes - Funded by HRSA and the Robert Woods Johnson Foundation, diabetes educational
classes are held at numerous sites around Galveston. Classes are designed to teach participants about the effects
of diabetes, how to deal with diabetes, and how to effectively live with diabetes. Classes are available in English
and Spanish. Attached is a schedule of our
education and cooking classes.

Friday Food Fair - The Jesse Tree hosts a food fair two Fridays per month on Galveston Island & Bolivar Peninsula, and the opposite Fridays in Texas City. Food is donated by the Houston Food Bank.

The Renaissance Zone Program - A grass roots program aimed
at improving all aspects of the community around the elementary schools in
Galveston. The City of Galveston Families, Children, and Youth Board is
presently working with Scott Elementary school.

Asset Map for Galveston County

We suggest visiting the Web Care Resources and Services Search Engine
(www.jessetree.net /assets) to identify Galveston community services using an asset map or a directory.
The “asset map” is a tool designed to visually organize community resources— organizations, people, and
agencies—that provide services related to a specific health or social need. For example, an asset map for
childhood immunization would identify all of the places where a child can be immunized and arrange them
according to selected characteristics such as cost, geographic location, or hours of operation. This tool
can be used by health and social service providers for information and referral or by consumers to identify
services that meet their particular needs. The Web Care Resources and Services Search Engine is a collaborative
project managed by the Jesse Tree and Coastal AHEC.

God's Fan Club - God's Fan Club provides electric fans, air conditioners and heaters to the elderly, homebound,
or physically disabled who do not have these items in their home. In addition, the staff and volunteers provide
comprehensive assessments to assist any individuals in need of social or medical services. The Jesse Tree convened
a committee of physicians, social service personnel, and senior citizen advocates to gather, compile, and disseminate
medical information regarding heat related illness and how to avoid medical complications.

Galveston County Immunization Coalition - This project is a County wide effort to improve the immunization rates in
Galveston County. Working collaboratively with the Galveston County Health District (GCHD), UTMB has partnered with
Galveston County school districts to immunize children. Vaccines are free of charge through the Vaccines for Children
Program and at the GCHD clinics no child will be turned away because they cannot pay. Please visit the
